LogMiddleware = (ctx, next) => void

Middleware function in the logging pipeline.

Receives the LogContext and a next callback. Mutate ctx.args (or other fields) then call next() to continue the pipeline. Calling next() is what forwards the log to output (and subsequent middleware).
